Rehab is a pain. So much so that only 10% of qualifying Cardiac and Pulmonary patients attend, which results in complications for patients and at least $190B in costs. At Carda Health, we’ve re-imagined rehab. Our program allows patients to complete engaging, compassionate, and life-saving rehabilitation remotely.

The Chronic Care Management (CCM) Coach plays a critical role in Carda Health’s programs by performing chronic care management services under the supervision of the nurse practitioner for patients who are participating in Phase 3 Pulmonary Rehab. This role encourages patients to reach their health goals while maintaining patient safety, evaluates medical appropriateness for exercise-based programs, is responsive to patient’s emergent medical needs, and supports the Carda Health team in maintaining a high standard of care aligned with clinical guidelines. Candidates should have experience in patient care in a clinical setting, be comfortable in emergencies situations, be comfortable with live video telehealth visits, have a thorough knowledge of common disease processes such as COPD, CAD, T2DM, be aware of therapies and medications involved with those diagnosis, feel comfortable with various tech platforms and be adaptable for fast paced changes in the clinical setting.

Manage a large panel of Medicare patients with chronic conditions through monthly clinical calls, coaching, care coordination, and compliant documentation to improve outcomes and reduce hospitalizations.
Coordinates nursing students’ clinical placements by communicating with students, developing clinical site and preceptor relationships, matching students to suitable opportunities, and maintaining accurate placement records.
Coordinates behavioral health services for patients with complex needs, managing referrals, care transitions, and barriers while monitoring engagement and escalating clinical concerns to the appropriate clinician.
Coordinate clinician support and core operational workflows, including communication triage, provider preference changes, onboarding, offboarding, patient transfers, and scheduling support for clinical leaders.
Coordinate intakes, family onboarding, virtual groups, and administrative workflows for an adolescent mental health program while supporting clinical teams and communicating with teens and families.
