ABOUT MATTER HEALTH

Matter Health delivers primary and preventive healthcare directly to Medicare-eligible adults living in affordable housing communities. Our mission is simple: to improve healthcare access by meeting patients where they are and building trusted relationships within the communities we serve. As we rapidly expand across the country, we are building a team of mission-driven professionals who thrive in fast-paced, collaborative environments.

We Lead with Love in Everything We Do.

ABOUT THE ROLE

Matter Health is seeking compassionate, dependable individuals to join our team as Care Facilitators. This seasonal, part-time, field-based position plays an important role in helping older adults access healthcare by connecting them with Matter Health providers through virtual visits.

As a Care Facilitator, you will travel to assigned residential communities, meet patients in their apartments at scheduled appointment times, assist them with completing consent forms on a company-provided iPad, and help connect them to a virtual appointment with a Matter Health provider.

This is a non-clinical support position. You will not provide medical care, make clinical decisions, conduct patient outreach, or be responsible for building a patient panel. Your focus will be ensuring patients feel comfortable, technology is working properly, and scheduled visits run smoothly.

No previous healthcare experience or clinical certification is required. We're looking for individuals who are reliable, patient, comfortable with technology, and passionate about making a positive difference in the lives of older adults.

RESPONSIBILITIES

  • Travel to assigned residential communities and arrive at patient apartments at scheduled appointment times.

  • Greet patients, verify their identity, and explain the virtual visit process in a clear, friendly, and professional manner.

  • Assist patients with reviewing and completing required consent forms using a Matter Health iPad.

  • Log into the virtual visit platform and connect patients with their Matter Health provider.

  • Confirm that the patient and provider can see and hear each other before stepping away to allow for a private appointment.

  • Return after the appointment to end the session, collect equipment, and confirm the visit is complete.

  • Follow a predetermined daily schedule and move efficiently between scheduled patient visits.

  • Communicate scheduling delays, patient availability concerns, or technology issues promptly to the Market Operations Leader.

  • Keep company-provided iPads and equipment charged, secure, and ready for use throughout the workday.

  • Document visit-related issues, including missed appointments, declined consent, or technology challenges, according to established procedures.

  • Maintain patient confidentiality and comply with HIPAA and Matter Health policies.

  • Complete required onboarding and training before beginning patient visits.

  • Demonstrate empathy, professionalism, and respect in every interaction with patients, providers, and community members.

QUALIFICATIONS

  • High school diploma or equivalent required.

  • Valid driver's license and reliable personal transportation required.

  • Ability to travel locally between assigned residential communities on a daily basis.

  • Comfortable using basic technology, including iPads, smartphones, mobile applications, and video calling platforms.

  • Strong verbal and written communication skills in English, with the ability to explain simple instructions clearly.

  • Dependable, punctual, and able to follow a structured daily schedule.

  • Ability to work independently while maintaining communication with the Market Operations Leader.

  • Strong interpersonal skills and a compassionate, patient-centered approach when working with older adults.

  • Good organizational skills and attention to detail.

  • Ability to successfully complete a background check.

PREFERRED QUALIFICATIONS

  • Experience working with older adults or in senior housing, affordable housing, home care, healthcare, hospitality, or customer service.

  • Familiarity with telehealth platforms, video calling, or basic technology troubleshooting.

  • Bilingual communication skills relevant to the communities served.

WHAT SUCCESS LOOKS LIKE

  • Arriving on time and completing scheduled patient visits efficiently and reliably.

  • Creating a welcoming, comfortable experience for older adults during their first virtual healthcare appointment.

  • Ensuring patient consent is completed accurately before each visit begins.

  • Connecting patients and providers successfully through the virtual visit platform.

  • Maintaining accurate visit documentation and promptly communicating challenges or delays.

  • Protecting patient privacy and maintaining professional boundaries.

  • Serving as a dependable, compassionate representative of Matter Health within the communities we serve.

WORK LOCATION & SCHEDULE

This is a seasonal, part-time, field-based position supporting Matter Health's initial patient visit program.

Care Facilitators work onsite at assigned residential communities and travel between properties throughout their scheduled shifts. This is not a remote or hybrid position.

Daily local travel using a personal vehicle is required as a condition of employment. Mileage is reimbursed at the IRS standard mileage rate, and employees operating personal vehicles on company business are covered under Matter Health's automobile liability policy.

The position requires frequent walking, standing, and navigating stairs in residential buildings where elevators may not be available. Employees must also be able to transport company-provided equipment weighing up to 10 pounds.

The duration of this seasonal assignment is based on operational needs and may be extended or ended depending on the needs of the initial patient visit program.

Compensation: $17.00–$20.00 per hour.

This is a seasonal, part-time position and is not eligible for Matter Health's standard full-time employee benefits package.

WHY MATTER HEALTH

We believe where someone lives should never determine the healthcare they receive. Every team member at Matter Health plays an important role in expanding healthcare access for older adults living in affordable housing communities.

As a Care Facilitator, you'll have the opportunity to make a meaningful difference by helping patients feel comfortable and connected during their healthcare visits. You'll receive the training and equipment needed to support patients while contributing to a mission-driven organization focused on removing barriers to care.

If you're dependable, enjoy helping others, and want to be part of a team making healthcare more accessible, we'd love to hear from you.

We Lead with Love in Everything We Do.

EQUAL OPPORTUNITY EMPLOYER

Matter Health is an equal opportunity employer committed to building a diverse and inclusive workplace. We welcome applicants from all backgrounds and experiences and are dedicated to creating an environment where every team member feels valued, respected, and supported.
