Transitions of Care Advanced Practice Provider

This position represents an amazing opportunity for an Advanced Practice Provider (NP/PA) to help build Devoted Medical’s Transitions of Care (TOC) Program.

Description

• Perform initial TOC visits including comprehensive post-discharge assessments.
• Complete post-discharge medication reconciliation and place appropriate orders.
• Provide disease-specific clinical education and communicate contingency plans.
• Conduct acute symptom TOC / virtual urgent care visits.
• Participate in interdisciplinary TOC rounds to discuss high-risk patients.
• Enhance team culture by facilitating safe communication across roles.

Requirements

• At least 3 years of experience in hospital, post-hospital, or primary care settings.
• Strong understanding of common hospital discharge diagnoses and post-discharge needs.
• Active RN and APRN licenses or Physician Assistant licenses in good standing.
• Active BLS certification at time of hire.
• Ability to work full-time, including one weekend shift per month.
