Manager, TOC Social Work Case Management

The Manager of Social Work Case Management will lead a high-performing team to support patients through transitions of care, ensuring effective management and improved outcomes.

Description

• Build and develop a high-performing team of Social Worker Case Managers and behavioral health care coordinators.
• Track staff performance through competency check-offs, audits, and coaching.
• Create workflows and plans of care to enhance team efficiency and patient results.
• Support the team on escalated patient needs and conduct direct patient outreach.
• Partner with cross-functional teams to standardize workflows across programs.

Requirements

• An active social work license (LMSW or higher).
• 3+ years of experience in case management.
• 3+ years of experience in people management.
• Experience working with interdisciplinary care management teams.
• Ability to manage a fully remote schedule.
