Your role:
Base salary range: €36,000 – €46,000 gross annual salary.

Coordinates hospital operations for a readmissions reduction program and provides behavioral health case management, including client intake, care coordination, crisis response, and discharge support.
Coordinate personalised home care for older Australians by assessing client needs, developing care plans, monitoring service delivery, and working with families, care professionals, and internal teams.
Provides one-to-one behavioral interventions for children and adolescents while coordinating cases, authorizations, documentation, and continuity of care across families, clinical teams, schools, and insurance providers.
Provide nonclinical case management for older adults and adults with disabilities, conducting home visits, educating clients and caregivers, coordinating care plans, and connecting clients with healthcare and community resources.
Own provider credentialing, onboarding, SOPs, compliance checks, and operational tooling for US clinical practice, building automated, auditable workflows that keep providers ready to practice as operations scale.
