Supervisor, Transitions of Care Coordinator

As the Supervisor of Transitions of Care Coordinators, you will lead a team to support patients in navigating healthcare transitions and managing chronic diseases.

Description

• Lead a team of TOC Coordinators to advocate for patients during transitions of care.
• Connect patients with necessary resources like appointments and transportation.
• Monitor team performance and provide coaching and support.
• Participate in cross-functional meetings to represent TOC coordination.
• Assist in the recruitment and onboarding of new TOC Coordinators.

Requirements

• MA Certification (desired), expired MA certifications are acceptable.
• 2 years of direct lead or supervisory experience in a virtual setting required.
• 2+ years healthcare administrative/office experience preferred.
• Proficient in technology using evolving platforms.
• Fully bilingual (English/Spanish) preferred but not required.
