- Follow Substance Use Disorder Program guidelines to plan care, assess client needs, and complete screenings.
- Refer clients to appropriate resources and coordinate with the Substance Use Disorder Program team and community agencies.
- Identify individual member needs and document interventions based on relationships and assessments.
- Provide support through various settings and assist with support groups as needed.
- Contribute to case management meetings to ensure a team approach for each member.

- Strong organizational skills and a talent for problem-solving.
