π Description
- Provide virtual primary care visits via telehealth, including comprehensive assessments and chronic disease management.
- Diagnose and manage chronic conditions and develop individualized care plans.
- Evaluate and treat acute concerns within the scope of virtual care.
- Order and interpret lab tests and coordinate follow-up care as necessary.
- Manage medication prescriptions and provide health coaching to empower patients.
π οΈ Requirements
- Active Nurse Practitioner (APRN) license in good standing.
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
- Minimum 2 years of primary care, urgent care, or internal medicine experience.
- Active BLS certification at time of hire.
