Role Summary
As a Care Navigation Care Coordinator, you will guide members and families through the healthcare system with empathy and clarity. You will partner with care managers and clinical teams to coordinate seamless transitions and resolve benefits questions. Your work ensures every member receives accessible, timely, and supportive care.

The actual pay rate offered within the range will depend on factors including geographic location, qualifications, experience, and internal equity. In addition to the hourly rate, you will be eligible for 5,000 stock options and benefits like health insurance, 401k, and paid time off. Coordinates care for pre-lung transplant patients across the care continuum, collaborating with clinicians, arranging testing and follow-up, and providing tailored education to patients and families.
Coordinate long-term care services by managing individualized service plans, documentation, appointments, and provider communication while helping clients and families access person-centered support.
Coordinate patients’ early care journeys by supporting activation and engagement, identifying barriers, and working with providers and facilities to arrange timely transitions to higher levels of care.
Build and strengthen relationships with clinical research sites, investigators, and healthcare organizations while identifying qualified collaborators, supporting site onboarding, and advancing network growth.
Coordinates site communications, clinical supplies, regulatory submissions, study documentation, training, safety information, and CTMS updates to support clinical research sites and study monitors.
