Weekend Everyday Care Advanced Practice Provider (NP or PA) - Responsive Care

This fully remote full-time position supports weekend clinical coverage with a 3x12-hour shift, requiring regular Saturday and Sunday work.

Description

• Conduct focused assessments of patients with chronic health conditions.
• Perform virtual visits, dedicating 40-60% of clinical effort to this task.
• Manage a refill inbox for patients in accordance with established protocols.
• Participate in centralized results management workflows.
• Leverage digital tools to enhance medication renewal and patient communication.

Requirements

• APP with three or more years in outpatient clinical practice.
• Minimum of 2 years focused on primary care or subspecialty with hypertension and lipid management.
• Active and in-good-standing RN and APRN licenses for Nurse Practitioners.
• Active and in-good-standing Physician Assistant license for PAs.
• Ability to obtain licensure in requested states within 90 days of hire.
