- Reach out to patients during their inpatient stay to introduce the TOC program and discuss post-discharge follow-up.
- Schedule post-discharge follow-ups with primary care and specialty providers.
- Obtain clinical documentation from hospitals and provider offices.
- Submit Home Care and DME orders and follow up with vendors.
- Monitor return fax queues and update provider contact information.
- Assist patients with technology preparation for telehealth visits.
🛠️ Requirements
- 2+ years of patient-facing and care coordination experience in healthcare, managed care, home health, or community health.
- Experience in a telemedicine environment.
- Fully bilingual in English and Haitian Creole.
- Managed Care / Health Plan experience is desired.
- MA Certification is preferred, expired certifications are acceptable.
