Position Summary

The HCBS Services Coordinator is responsible for supporting the delivery and coordination of Home and Community-Based Services (HCBS) for members. This position works closely with members and families, Direct Care Workers (DCWs), case management staff, and the HCBS team to ensure appropriate staffing, onboarding, service coordination, and ongoing monitoring. This position allows for about 8 hours a week to start. Must be within Arizona.

Essential Responsibilities

DCW Recruitment & Hiring

  • Recruit and hire Direct Care Workers (DCWs) to support DDD members.
  • Coordinate the hiring process and communicate with prospective DCWs throughout recruitment.
  • Assist in identifying appropriate DCWs based on member staffing needs.

DCW Onboarding

  • Complete DCW onboarding in collaboration with HCBS Compliance Coordinator.
  • Ensure required onboarding documentation and processes are completed prior to service delivery.
  • Communicate with new DCWs regarding onboarding requirements and next steps.

Member Staffing & Coordination

  • Coordinate DCW staffing needs with DDD members and families.
  • Work with Patient Services and Operations Coordinator on case building and staffing coordination.
  • Communicate with members and guardians regarding staffing needs and changes.
  • Assist with resolving staffing gaps and coordinating appropriate DCW coverage.

PCSP Meetings

  • Attend Person-Centered Service Plan (PCSP) meetings as appropriate.
  • Participate in discussions regarding member needs, services, staffing, and service delivery.
  • Communicate relevant information from PCSP meetings to the HCBS team.
  • Follow up on action items identified during PCSP meetings.

ATC Monitoring Meetings

  • Conduct ATC Monitoring Meetings every 90 days for DDD members.
  • Schedule ATC Monitoring Meetings with members, guardians, and applicable team members.
  • Conduct and document ATC Monitoring Meetings.
  • Review service delivery and address questions or concerns identified during monitoring.
  • Complete required documentation following each meeting.
  • Follow up on outstanding items or concerns identified during ATC Monitoring Meetings.
  • Escalate significant concerns to the HCBS Director when appropriate.

Collaboration

The HCBS Services Coordinator will work closely with:

  • Admin Team Members
    • Billing Director
    • HCBS Compliance Coordinator
      • DCW Onboarding
    • Patient Services and Operations Coordinator
      • DDD Member case building and staffing coordination
  • Direct Care Workers
  • Members and guardians
  • Support Coordinators and other members of the care team

Qualifications

  • Attendance and participation in required PCSP meetings.
  • ATC Monitoring Meetings completed every 90 days.
  • Accurate and timely ATC documentation and follow-up.
  • Clear communication with members, families, DCWs, and the HCBS team.
