Deliver preventive telehealth care to Medicare patients, managing routine visits and chronic conditions while coordinating with an integrated care team to support healthier aging and reduce avoidable complications.
Provide clinical leadership for a growing care navigation program by evaluating members, overseeing care plans, responding to clinical escalations, and developing protocols and standards.
Provide short, on-demand video consultations, offering timely and accurate medical advice to patients through a secure digital platform while choosing when to accept consultations.
Provide evidence-based telehealth care to LGBTQAI+ members, including gender-affirming hormone therapy, sexual and reproductive health, primary care, and mental health services.
Provide virtual, evidence-based psychiatric care for children and adolescents through evaluations, diagnoses, individualized treatment plans, and medication management, partnering with families and care teams.
